Services

By facilitating opportunities for social activities, vocational training, and community involvement, we ensure individuals integrate seamlessly into their communities and lead fulfilling lives. Our commitment is to enable individuals with I/DD to become active, engaged members of society, contributing to the diversity and richness of their communities.


Homemaker Personal Care

Homemaker/Personal Care, often called HPC, supports a person to be more independent while meeting their daily living needs. Direct support providers help people with household chores and personal care, including things like: • getting dressed and keeping clean • making food • managing money to pay household bills • cleaning and doing laundry • getting out and being a part of the community • helping people get to medical and dental appointments or other health care services.

HPC also includes support that improves a person’s ability to express their opinions and choices.


Non-Medical Transportation

Transportation to get to, from, between, or among: a place of employment • a location where adult day support is provided • career planning • group employment support • individual employment support • vocational habilitation is provided • a volunteer activity • post-secondary educational program • an internship or practicum • drop-off or transfer location from which the individual is then transported to or from one of above listed places.


Transportation

Used for community activities, accessing waiver services, and completing daily living tasks. This is typically used in conjunction with homemaker personal care services to get to and from activities with a provider.


Shared Living

Ohio Shared Living provides an option for adults with a developmental disability to get the support they need while living with a family member or a caregiver. The person resides with one or more family members or caregivers, and they provide twenty percent or more of the person's care and support services. Shared Living family members and caregivers support adults with developmental disabilities in a home setting.

  • provides consistency of care and a safe environment;
  • helps the person experience a genuine community life;
  • develops life routines chosen by the person; and
  • promotes shared decision-making


Residential Living

Only Residential respite at the daily rate can be provided in an ICF (intermediate care facility), but both the daily rate and 15-minute unit of rate residential respite can be provided in a non-ICF licensed Residential Facility, a person’s home, the home of an employee of the agency provider, the home of the independent provider or another location chosen by the person.
